When you come to our office, we are sorry that we must ask you about your insurance coverage up front. We would certainly prefer to concentrate only on patient care. Changes in the insurance world mean we now have responsibility to your insurance company. We may be required to have primary care provider referrals, do pre-authorization for certain procedures, etc. In order to meet these insurer requirements, we have no choice but to get information from you before you are seen. If you are covered by an insurer not listed above, payment is expected at the time of service. You will be provided with billing information to submit to your insurance company.
